I understand that CVRx may share this information with its business partners or potential study center(s) so that they may contact me to see if I am a suitable candidate for the Rheos Study and, if appropriate, to assist me in enrolling in the Study. I understand that CVRx cannot guarantee that I will be contacted after I submit my personal information. I further understand that CVRx is not responsible for how the study center or other third party handles my personal information.
I verify that the name and phone number I have provided belongs to me and is correct. I further verify that I am the interested candidate for the Rheos Study, or if I am not the candidate for this Study, I am the caregiver, legal guardian, or parent of the potential candidate. As caregiver, guardian, or parent, I understand that I must provide the contact information for myself, and I will be the primary contact for the person requesting involvement in the Study, to the extent permitted by state law.

Thank you for your interest in the Rheos System clinical study. This web site (the “Site”) is owned and operated by CVRx, Inc., the sponsor of the Study. This Privacy Policy (“Policy”) describes the information CVRx may collect when you use the Site and how we may use the information collected. By using this Site, you agree to the terms of the most recent version of the Policy. This Policy forms part of our Site terms and conditions and as such shall be governed by and construed in accordance with the laws of the State of Minnesota.
"Personal information" means information about an identified or identifiable individual. Your Personal Information may include your name, your address, and your e-mail address. The only Personal Information CVRx currently collects through this Site is the information you knowingly and voluntarily give us when you use our Site. For example, in order to receive more information about the Rheos Study, you may provide your name, zip code, e-mail address and phone number.
The term “Anonymous Information” refers to information we collect through this Site that does not identify you as an individual, such as the name of the domain from which you access the internet, the date and time you access our Site and which pages you have visited. If you visit our Site to read or download information, we may collect certain Anonymous Information about you from your computer. This information is collected from your computer's web browser. If you only read or download information on our Site, we do not collect or learn your name, e-mail address, or other Personal Information about you.
CVRx limits the collection and processing of information to what is necessary to fulfill the purposes for which it is to be used.
Because Anonymous Information cannot identify you or be tied to you in any way, there are no restrictions on the ways that we can use or share Anonymous Information. CVRx may use Anonymous Information and share it with third parties to improve the content and functionality of our Site, to better understand our customers and markets, and to improve our products and services.
Personal Information may be kept and used in the following ways:
- To contact you regarding potential eligibility for participation in the Rheos Study;
- To develop records, including records of your Personal Information;
- For analytical purposes and to research, develop and improve programs, products, services and content, including the content and functionality of our Site;
- To remove your personal identifiers (your name, e-mail address, etc.). In this case, you would no longer be identified as a single unique individual. Once we have de-identified information, it is Anonymous Information and we may treat it like other Anonymous Information;
- To enforce this Policy and other rules about your use of this Site;
- To protect our rights or property;
- To protect someone's health, safety or welfare.
In addition to the ways we may keep, disclose and use information that are specifically described in this Policy, CVRx may keep, disclose and use Personal Information you give us through this Site in any way we believe is necessary to comply with a law, regulation, governmental guidance, court order or other legal process.
CVRx may share the information you provide with its affiliates, business partners, agents or potential study center(s) so that they may contact you to see you are a suitable candidate for the Rheos Study and, if appropriate, to enroll you in the Study. In the ordinary course of business, CVRx may also share some Personal Information with companies that we hire to perform services or functions on our behalf. In all cases in which we share your Personal Information with a third party, we will not authorize them to keep, disclose or use your information with others except for the purpose of providing the services we asked them to provide.
CVRx may also share or transfer Personal Information in the event of a corporate sale, merger, reorganization, dissolution or similar event. We may disclose your Personal Information to third parties if we feel that the disclosure is necessary to enforce this Policy, to protect someone’s health, safety or welfare or to protect our rights or property. We may cooperate with law enforcement authorities in investigating and prosecuting Site visitors who violate our rules or engage in behavior which is harmful to other visitors (or illegal). In addition, as described above, we may be legally compelled to release your Personal Information in response to a court order, subpoena, search warrant, law or regulation.
CVRx is continually improving and updating this Site. Because of these ongoing changes, changes in the law and the changing nature of technology, CVRx’s data practices will change from time to time. CVRx may, in its sole discretion, update this Policy at any time, by posting the amended Policy on this Site. The amended Policy shall be effective upon posting, and your access to or use of the Site following an update shall be deemed consent to the revised Policy. We encourage you to check this page frequently, and especially prior to providing or updating any Personal Information.
This Policy applies only to the Rheos Study Site. This Site may also contain links to other web sites, including the CVRx website, which in turn may provide links to other third party websites. We provide any such links for your convenience but we do not review, control, or monitor the privacy practices of web sites operated by third parties. This Policy does not apply to any other web site, including but not limited to the CVRx web site. We are not responsible for the performance of web sites operated by third parties or for your business dealings with them. Therefore, we urge you to review the privacy practices for any web site you visit before using that site or providing any Personal Information about yourself.
CVRx will not intentionally collect any Personal Information (such as a child’s name or e-mail address) from children under the age of 13. If you think that we have collected Personal Information from a child under the age of 13, please contact us.
We take reasonable steps to protect your Personal Information from loss, misuse and unauthorized access, disclosure, alternation, or destruction. However, no internet transmission is ever 100% secure or error-free. In particular, e-mail may not be secure and you should therefore take special care in deciding what information you send to us via e-mail.
